Report an error or request removal
This directory is built from a government dataset that goes stale between updates. If something is wrong, we would rather hear it than leave it up.
What to send
- The facility name and the page URL.
- What is wrong: closed, moved, wrong phone number, service no longer offered, wrong insurance.
- The correct information, if you have it.
- Whether you work for the facility (helpful, not required).
Email corrections@example.com. Corrections are applied within five business days, and removal requests from a facility are honoured without argument.
